2010 BELLA VISTA ATHLETIC BOOSTER CLUB
SCHOLAR-ATHLETE SCHOLARSHIP
Print and complete a copy of each page, and turn in by the due date and time.
Completed applications must be returned to Mrs. Shenoy in Administration by 3:00 pm on MONDAY, APRIL 12th 2010.
ALL APPLICATIONS MUST BE PLACED IN AN ENVELOPE ADDRESSED TO BELLA VISTA ATHLETIC BOOSTER CLUB SCHOLARSHIP. PLEASE INCLUDE ALL PAGES OF THIS SCHOLARSHIP IN YOUR ENVELOPE. NO EXCEPTIONS WILL BE MADE FOR LATE OR INCOMPLETE APPLICATIONS.
WE URGE ALL QUALIFIED STUDENTS TO APPLY!

Remember also that 2 years membership in the BV Athletic Booster Club or a Life Membership (by April 1, 2010) is needed before submitting the scholarship application, as these scholarships are intended for the students and families who have supported the BV Athletic Booster Club with their membership.
